We are requesting a one-time grant to expand our classroom libraries.  Studies show that children from low income families have fewer books available to them at school.

Community Children Health Access/LVJUSD
Purpose is to assist to remove barriers to success in school by providing access to basic daily needs such as food, clothing, andtransportation and to provide access to basic daily needs such as food, clothing, and transportation and to provide assistance with local medical, dental and optometric care for low income, uninsuredand underinsured  school age children.  This program addresses the needs of students that have difficulty learning because they are hungry, physically ill, have dental problems, or have vision impair-ments.  This program reaches out to all students in the LivermoreValley Joint Unified School District to remove barriers to care.

CALICO Child Abuse Intervention
Project Title:  Livermore Child Abuse Intervention ProjectWe are requesting a grant to provide a skilled, multi-disciplinary response toLivermore toddlers, children and adolescents who suffer sexual or physical abuseor other harm.  Through the Livermore Child Abuse Intervention Project, CALICO aims to conduct forensic interviews with 40 Livermore victims and provide on-siteand follow-up support services to approximately 30 caregivers (in FY 13k-14).  Wewill facilitate a multi-disciplinary team to respond to child abuse allegations that will involve the Livermore Police Department (LPD), child protective services, the District
Attorney’s Office, Children’s or Highland Hospital, mental health service providers and
other partners as needed to ensure the safety, health and well-being or each child and
his/her family.

Horizons Counseling Internship
The purpose is to provide individual therapy to at-risk middle school students in the Livermore Valley Joint Unified School District with a goal of improving the student’s success at school, behaviorally, academically, socially and emotionally. Funds to be used between July 1, 2013 to June 30, 2014

AXIS Community Health Systems
 New Autoclaves for Equipment Sterilization in Medical ClinicsThe purpose of this grant is to purchase two new autoclaves to replace the current ones that have been in use for over ten years.  Axis is the sole provider of preventive and primary medical services for low-income and uninsured families in the Tri-Valley and primary medical services for low-income and uninsured families in the Tri-Valley. Our request is to fund the Livermore Weekend Boxed Lunch Program.  The funds would focus on Livermore children in need.  The funding will provide food and supplies for approximately 5,000 boxed lunches out of the 82,000 boxed lunches to children of low income Livermore families.  The lunches will be distributed to children of low income Livermore families.  The lunches will be distributedFriday afternoons, to schools in Livermore in the 2012-2013 school years.
